When you submit the consultation form, we collect only the information you provide: your name, email address, phone number, meeting preference, optional age range, optional coverage interest, optional preferred contact time, any message you write, and your consent selection. We also record the page the form was submitted from and the date and time of submission.

We do not ask for and you should not submit Social Security numbers, driver's license numbers, financial account numbers, or detailed medical history through this website. Information needed for an insurance application is collected separately through the insurance company's secure application process.

How we use it

Your information is used to respond to your request, schedule and conduct an initial consultation, and, if you choose to move forward, to help you apply for coverage. We do not sell your personal information, and we do not share it with third parties for their own marketing purposes.

Who may see it

Submissions are stored in a secured database that is not publicly readable. Access is limited to Luke Simonetti and the service providers that host this website and its database. If you proceed with an application, information you provide at that stage is shared with the insurance company you apply to.

Communications and consent

By checking the consent box you agree to be contacted by phone, text, or email at the contact information you provided. Consent is not a condition of purchase. Message and data rates may apply. You may opt out at any time by replying STOP to a text, using the unsubscribe option in an email, or contacting us at lukesimonetti@wwwlsli.com.

Retention and your choices

We keep consultation requests only as long as needed for business and recordkeeping purposes. You may request a copy of the information you submitted, ask for corrections, or ask us to delete it, by contacting us at lukesimonetti@wwwlsli.com or 304-780-3344.

This website does not use advertising cookies or third-party tracking pixels. Basic technical logs may be kept by our hosting provider for security and reliability.

Children

This website is intended for adults and is not directed to children under 13.
